Dear Parents and Prospective Students,

Admission Process

To begin the Application process, you will be asked to fill out several forms:

1. NYS Commissioner’s Appointment Form
2. Release of Information Form
3. Case History Form

We will need a copy of your child’s most recent unaided audiogram.

If you do not have completed recent evaluations within the year for your child, we will schedule an appointment for admissions evaluations which may include:

• Audiological Testing
• Occupational Therapy Testing
• Physical Therapy Testing
• Psycho-Educational Testing
• Speech, Language & Communication Testing

The Multidisciplinary Evaluation Team will review all documents and make a recommendation.

If your child is admitted to SFDS, he/she will be attending a school that is committed to the academic development of all our students.  In a diverse, multimodal environment, we support each student to achieve personal excellence and become responsible lifelong learners. We accomplish this through the individualized Education Plan (IEP) process, where teachers and support staff strive to develop and implement an educational program designed to promote and develop every student’s educational need.
